If you reside in Miami-Dade or Broward Counties and have diabetes, you may qualify for therapeutic shoes and inserts. Medicare provides coverage for these extra-depth shoes for individuals who meet the criteria, as they are deemed essential for diabetic care.
Obtaining the correct documentation from your physician is crucial in this process. This documentation should include a written prescription as well as a comprehensive foot examination conducted by a qualified professional, such as an MD or DO. See attached forms to submit to your doctor.

Medicare Part B (Medical Insurance) does provide coverage for therapeutic shoes and inserts for individuals with diabetes who have a ‘qualifying’ condition as mentioned below. This coverage typically includes one pair of extra-depth shoes and three pairs of multi-density inserts per calendar year. Certain deductibles, co-insurances, and/or co-payments may apply.
It’s important to note that patients must meet specific criteria to be eligible, and the shoes or inserts must be prescribed by either a podiatrist or another qualified doctor, and provided by a podiatrist, orthotist, prosthetist, or pedorthist.

To qualify for diabetic shoes through Medicare, an individual must meet the following criteria:
- The patient has diabetes.
- The patient has at least one of the following conditions in one or both feet:
- Partial or complete foot amputation.
- History of foot ulceration.
- Foot deformity.
- Poor circulation.
- Calluses that could lead to ulcers.
- Neuropathy with a history of callus formation.
- The patient is under comprehensive diabetes care and requires therapeutic shoes and/or inserts due to diabetes.
- The shoes are prescribed by a podiatrist or another qualified doctor.
- The shoes or inserts are provided by a podiatrist, orthotist, prosthetist, or pedorthist.
Patients are required to have a doctor’s visit within three months from the time the shoes are dispensed, addressing the qualifying condition and performing an in-person evaluation. The prescription should be written during this particular visit. It is always best to check the most current eligibility guidelines directly with Medicare or consult a healthcare provider.
Medicare Part B coverage for therapeutic diabetic shoes includes one pair of shoes per calendar year. This can be either one pair of custom-molded shoes (if medically necessary) and inserts, or one pair of depth-inlay shoes and inserts.
Additionally, Medicare covers two extra pairs of inserts each calendar year for custom-molded shoes and three pairs of inserts each calendar year for extra-depth shoes.
